Abstract
Žmogaus papilomos viruso (ŽPV) infekcijos yra daugelio piktybinių ir nepiktybinių odos ir gleivinių ligų rizikos faktorius. Yra įrodyta, kad didelės rizikos (DR) ŽPV gali sukelti gimdos kaklelio vėžį. ŽPV infekcijos nustatymo efektyvumo padidinimui klinikinės diagnostikos laboratorijos dažnai šalia PAP testo (15% iki 30% šio testo atsakymų yra klaidingai neigiami) naudoja ir PGR metodą. Tyrimo metu buvo optimizuota ir įvertinta nauja 19 specifinių pradmenų PGR sistema (DR-ŽPV PGR), turinti vidinės kontrolės amplikoną. DR-ŽPV PGR sistemos analitiniams parametrams įvertinti buvo sukurta rekombinantinių plazmidžių biblioteka, sudaryta iš 17 DR ir 9 mažos rizikos (MR) plazmidžių. Rekombinantinių plazmidžių specifiškumas patvirtintas sekoskaita. Nustatyta optimali naujos DR-ŽPV PGR sistemos sudėtis. Jos efektyvumas įvertintas analizuojant klinikinius mėginius ir gautus rezultatus lyginant su duomenimis, gautais analizuojant tuos pačius mėginius su šiuo metu dažniausiai naudojamomis ŽPV PGR sistemomis su pradmenimis: GP5+/6+ ir PGMY09/11. Patikrinus 466 klinikinius mėginius, ŽPV infekcijos nustatymo efektyvumas siekė 61.3% su (GP5+/6) PGR, 47.6% - su (PGMY09/11) PGR ir 70.8% DR-ŽPV PGR. DR-ŽPV PGR sistemos jautrumas buvo įvertintas 17-ai DR ŽPV genotipų ir gauti rezultatai palyginti su „Abbott RealTime High Risk HPV“ deklaruojamais jautrumais. DR-ŽPV PGR sistemos įvertinimui buvo panaudoti citologiškai ir histologiškai įvertinti klinikiniai mėginiai su CIN3 diagnoze. Gauti rezultatai: su GP5+/6+/PGR ŽPV infekcija nustatyta 65.7%, su PGMY09/11/PGR 45.7%, o su naujai sukurta DR-ŽPV PGR – 100% tirtų mėginių. DR-ŽPV PGR sistemos jautrumas buvo vertintas panaudojant klinikinius mėginius su nustatyta ŽPV infekcija, naudojant reagentų rinkinius: 1) „Abbott RealTime High Risk HPV“; 2) Innogenetics kompanijos reagentų rinkinį - „Inno-Lipa HPV typing“. Gauti rezultatai: DR-ŽPV PGR sistema nustatė 56 infekuotus mėginius iš 56 (100% , Abbott sistema) ir 9 iš 10 (Inno-Lipa sistema). DR-ŽPV PGR sistemos kryžminis reaktyvumas su kitomis urogenitalinėmis infekcijomis nenustatytas.
Human papillomaviruses (HPV) infections are the major factor of many malignant and non malignant skin and mucous lesions and diseases. Now are clearly indicated that high risk (HR) HPV are the basic cause of cervical Cancer. To improve the efficiency of detection of HPV infections diagnostic laboratories still apply PCR procedures in parallel with standard cytological PAP (degree of false negative results is about 15-30%). During current research new PCR system based on set of 19 primers specific for HR HPV and internal control was optimized and evaluated. Two libraries of recombinant plasmids with loci of L1 genes of HR and low risk (LR) HPV were developed and used to evaluate analytical parameters of new HR-HPV PCR. The specificity of HPV DNA-containing plasmids was confirmed by sequence analysis. New HR HPV PCR was optimized according to components concentrations, PCR enhancers and PCR protocol. The efficiency of detection HPV infections of newly developed HR HPV was compared with the efficiency of the most popular PCR systems for HPV detection: PGMY09/11-PCR and GP(5+/6+)- PCR. DNA samples extracted from cohort of 466 clinical samples were tested and efficiency (number of HPV positive samples) of new HR HPV was 70.8%, GP(5+/6+)-PCR – 61.3% and PGMY09/11-PCR – only 47.6%. HR HPV DNA-containing plasmids were used to evaluate sensitivity of new HR-HPV PCR for 17 HR HPV strains and obtained results were compared with the sensitivity declared by „Abbott RealTime High Risk HPV“ kit. The efficiency of new HR HPV to detect infections was evaluated in cohort of 38 clinical samples validated by cytological and histological procedures as CIN3/Ca/IS. New HR HPV PCR detected infections in 100% of samples, PGMY09/11-PCR in 45.7% and d GP(5+/6+)-PCR – in 65.7% of samples. Next collection – clinical samples tested as positive using „Abbott RealTime High Risk HPV“ and “Inno-Lipa HPV typing“ kits were used in HR HPV PCR. New HR-HPV PCR detected viral infection in 56 of 56 Abbott tested samples and in 9 of 10 InnoLipa tested samples. Crossreactivity of HR HPV was not detected in clinical samples infected by pathogenic microorganisms of other sexually transmitted diseases.
