Sergančių vėžiu moterų gyvenimo kokybė ir jos kitimai gydantis stacionare

Gyvenimo kokybės tyrimai įgyja vis didesnę reikšmę kuriant ir tobulinant psichologinę pagalbą sergantiesiems vėžiu. Šiame darbe buvo siekiama įvertinti ginekologiniu vėžiu sergančių moterų gyvenimo kokybę ir jos kitimus gydantis stacionare priklausomai nuo vėžio stadijos, subjektyviai vertinamos sveikatos, amžiaus, išsilavinimo ir prisitaikymo prie aplinkos sunkumų. Palyginimui pateikiami vėžiu nesergančių moterų, bet besigydančių tame pačiame skyriuje, rezultatai. Keliama hipotezė, jog gyvenimo kokybė yra prastesnė ir ji kinta neigiama linkme tarp tų vėžiu sergančių moterų, kurios yra vyresnės, žemesnio išsilavinimo, turi vėlesnes vėžio stadijas, savo subjektyvią sveikatą vertina prasčiau. Gyvenimo kokybei vertinti buvo naudojamas Pasaulinės sveikatos organizacijos Gyvenimo kokybės sutrumpintas 26 klausimų klausimynas PSO-GK trumpas (THE WHOQOL – Bref Quality of Life Questionnaire). Lietuviškas klausimyno variantas parengtas 2000 m. N. Goštautaitės Midttun ir A. Goštauto adaptuoto WHOQOL-100 klausimyno pagrindu. Šis klausimynas pasirinktas dėl to, kad jis parengtas tyrimams Lietuvoje ir yra sėkmingai naudojamas. Pasaulinės sveikatos organizacijos Gyvenimo kokybės klausimynas PSO-GK trumpas leidžia įvertinti 4 gyvenimo kokybės sritis: fizinę sveikatą, psichologinę savijautą, socialinius santykius ir aplinką. Buvo rasta, kad sergančioms vėžiu moterims būdingi žemesni gyvenimo kokybės fizinės ir psichologinės sričių įvertinimai. Nepastebėta, kad sergančių vėžiu moterų gyvenimo kokybė ir jos kitimai patikimai susiję su amžium ar išsilavinimu. Sergančių vėžiu moterų, kurių subjektyviai vertinama sveikata geresnė, gyvenimo kokybė patikimai aukštesnė trijose srityse: fizinės sveikatos, psichologinėje ir socialinių santykių. Nepastebėta, kad gydantis stacionare sergančių vėžiu moterų, kurių subjektyviai vertinama sveikata prastesnė, gyvenimo kokybės sričių pokyčiai bus neigiami. Moterų, sergančių III ir IV vėžio stadija, gyvenimo kokybės aplinkos srities įvertinimai gydantis stacionare prastėja. Geresnis prisitaikymas prie aplinkos statistiškai patikimai siejasi su geresne moters, sergančios vėžiu, gyvenimo kokybe fizinės sveikatos, psichologinėje ir socialinių santykių srityse. Bet teigti, kad gyvenimo kokybės kitimai siejasi su geresniu ar prastesniu moters prisitaikymu prie aplinkos negalime.
The quality of life researches gain the meaning creating and improving psychological assistance for people with cancer. Here we aim to evaluate the quality of women’s life who have gynecological cancer and changes in the quality of life during inpatient treatment in regard to different cancer stages, subjective health evaluation, age and education. For comparison purposes there is given women’s quality life results who do not have cancer, but also have inpatient treatment. It is thought that quality of life is worse and changes are negative among women who are in later stages, who have worse subjective health evaluation, are older or among women who have lower education. In order to evaluate the quality of life for the purposes of this research there was used a life quality questionnaire of World Health Organization - THE WHOQOL – Bref Quality of Life Questionnaire. It was chosen because it is prepared for researches in Lithuania (by N. Goštautaitė Midttun and A. Goštautas, 2000). THE WHOQOL – Brief Quality of Life Questionnaire allows evaluating 4 main domains of the quality of life: physical health, psychological, social relationships and environment. It was found that women with cancer have worse evaluation in physical health and psychological domains. Age and education are not statistical related to quality of life and its changes of women with cancer. Women with cancer who have better subjective health evaluation the quality of life is significant higher in physical health, psychological and social relationships domains. It is not seen, that women with cancer who have worse subjective health evaluation have negative changes in the quality of life during inpatient treatment. Women with later cancer stages have the significant negative changes in environment domain. Better adaptability is statistical related to better quality of women’s life with cancer in physical health, psychological and social relationships domains. We can not maintain that changes in quality of life significant related with better adaptability.
