Priešlaikinį gimdymą patyrusių moterų su gimdymu susijusios patirtys COVID-19 kontekste
Gimdymas yra vienas iš reikšmingiausių gyvenimo įvykių moteriai ir jos šeimai. Šis įvykis stipriai veikia moters fizinę ir psichinę sveikatą. Priešlaikinis gimdymas yra vienas iš galimų trauminių įvykių moters gyvenime, o su COVID-19 taikomi apribojimai ligoninėse galėjo šią patirtį dar apsunkinti. Tyrimo tikslas - atskleisti priešlaikinį gimdymą patyrusių moterų patirtis COVID-19 kontekste. Tyrime dalyvavo 10 moterų, priešlaikinį gimdymą patyrusių prieš 2-9 mėn. ir tai buvo jų pirmasis gimdymas. Atliktas kokybinis tyrimas, duomenų rinkimui panaudojant pusiau struktūruoto interviu metodą. Duomenys analizuojami pasitelkiant indukcinę teminę analizę. Remiantis tyrime dalyvavusių pirmą kartą gimdžiusių, priešlaikinį gimdymą turėjusių moterų patyrimo analizės rezultatais, atskleistos penkios visoms būdingos patyrimo temos:1) Pirmosios reakcijos susidūrus su priešlaikiniu gimdymu; 2) Pogimdyvinio laikotarpio iššūkiai; 3) Priešlaikinio gimdymo poveiki; 4) Patyrimo ligoninėje vertinimas; 5) Tai, kas tuo metu padėjo. Tyrimas atskleidė, kad susidūrusios su priešlaikiniu gimdymu moterys išgyvena pasimetimo jausmą, joms būna sunku suvokti, kas tuo metu vyksta. Moterys taip pat jaučia baimę, dėl to kas jų laukia, dėl vaiko sveikatos, išgyvenamas nežinomybės jausmas. Kartais kyla kaltės jausmas, užduodamas klausimas "kodėl man taip nutiko"? Pogimdyviniu laikotarpiu moterys ir toliau susiduria su sunkiais išgyvenimais ir iššūkiais. Dažnai moterims sunku prisiimti naują mamos vaidmenį, vis dar kyla baimė dėl kūdikio sveikatos, galimų komplikacijų. Moterys jaučia beviltiškumo jausmą, dėl to, jog tuo metu jos niekaip negali padėti savo kūdikiui. Taip pat susiduriama su iššūkiais susijusiais su vaiko lankymu intensyviosios priežiūros skyriuje, bei naujagimio maitinimu. Priešlaikinis gimdymas sukelia baimę gimdyti ateityje bei pakeičia požiūrį į neišnešiotukus bei panašius išgyvenimus patyrusias mamas. Priešlaikinį gimdymą moterys įvardina kaip traumuojančią patirtį, tačiau motinystė bei kūdikio gera būklė paskatina susitaikymą su tokia patirtimi.Moterys ligoninėje pasigenda informacijos apie kūdikio būklę, tyrimus bei patį priešlaikinį gimdymą. Pasigendama ir tinkamos psichologinės pagalbos joms sunkiu metu. Įvardijamas neempatiško personalo poveikis jų emocinei sveikatai bei dėl COVID-19 apribojimų jaučiamas vienišumas ligoninėje. Darbas su savo mintimis, stengiantis atrasti savyje stiprybių, pozityvus mąstymas ar tam tikras atsiribojimas nuo blogų emocijų palengvina moterų išgyvenimus. Padeda bendravimas su kitomis priešlaikinį gimdymą patyrusiomis moterimis, artimųjų palaikymas ir sulaukta pagalba iš personalo.
Childbirth is one of the most significant life events for a woman and her family. This event has a strong impact on a woman’s physical and mental health. Premature birth is one of the possible traumatic events in a woman’s life, and restrictions on the hospitals related to COVID-19 may have complicated this experience even more. The aim of the study: to reveal the experience of women who have experienced preterm birth in the context of COVID-19. This study involved 10 women who had experienced preterm birth 2-9 months ago, and this was their first time giving birth. A qualitative study was conducted using a semi-structured interview method for data collection. Inductive thematic analysis was used in the analysis. Based on the results of the analysis of the experience of women who gave birth for the first time and had a premature birth, five general topics of experience were revealed: 1) First reactions facing preterm birth; 2) Postpartum challenges; 3) Impact of preterm birth; 4) Evaluation of hospital experience; 5) What helped at the time. The study found that women who experience preterm birth report experiencing a feeling of confusion, find it difficult to understand what is happening at that time. Women also feel fear and uncertainty, about the future and their child’s health. Sometimes they feel guilt and start asking the question “why did this happen to me”? In the postpartum period, women continue to face difficult experiences and challenges. It is often difficult for women to take on the new role of mother, there are still fears about the health of the baby, possible complications. Women feel a sense of despair because they can’t help their baby in any way at the time. There are also challenges related to visiting a child in an intensive care unit and feeding a newborn. Preterm birth causes fear of giving birth in the future and changes attitudes towards premature babies and mothers who have experienced similar experiences. Premature childbirth is described by women as a traumatic experience, but motherhood and the good condition of the baby encourage reconciliation with such experiences.Women in the hospital lack information about the baby's condition, tests, and the premature birth itself. They also lack proper psychological help during difficult times. The effects of non-empathic staff on their emotional health are identified, and the limitations of COVID-19 make them feel lonely in a hospital. Working with your thoughts to discover strengths within yourself, positive thinking, or some detachment from bad emotions facilitates women’s experiences. Also helps communication with other women who have experienced preterm birth, support from loved nes, and assistance from staff.
